PASSPORT BOOKS & PASSPORT CARDS
We are an authorized passport agent. Passport Books & Passport Cards can be processed in our office Monday-Friday 8:00a.m. - 4:30p.m.
Passport Books may be used for any type of international travel. Passport Cards may only be used for re-entry into the United States at land borders and sea ports-of-entry when traveling from Canada, Mexico, the Caribbean and Bermuda. Passport Cards may not be used for air travel.
When you come in to the office be sure to have your 2 passport photos, a State Certified Copy of your birth certificate (or old passport) and a photo ID. Please note that the Hospital Birth Certificates are not acceptable. Your photos and birth certificate will be submitted with your application. Your birth certificate will be returned to you with your passport.

Your passport application should be completed entirely with the exception of signing it. Do not sign until you can do so in front of an authorized passport agent. It takes 4-6 weeks for your passport to be processed.

CHILDREN UNDER 16 YEARS OF AGE (DS-2053)

If applying for children under 16 years of age, both parents need to come in with the children to sign the application. Parents need to bring Photo ID with them. If one parent cannot come in to sign, there is a form that needs to be filled out & signed in front of a Notary.
EXPEDITED SERVICE
If you need your Passport Book sooner than 4-6 weeks, you can have your application expedited for an extra $60.00 for expedite fee and $14.85 for overnight return delivery service of the Passport Book Only. This fee is in addition to your regular passport fee. This fee needs to be either a personal check or money order. You will need to go to the post office and bring back with you one paid overnight mail envelope to send your expedited request. This will get your Passport Book back to you within two weeks. Expediting Passport Card applications would also be an extra $60.00 in addition to your regular Passport card fee. You will need to go to the post office and bring back with you one paid overnight mail envelope to send your expedited request. Passport cards cannot be sent to customers using overnight delivery service.
PASSPORT RENEWAL BY MAIL (DS-82)
(Customer submits)
You may qualify to use this form only if:
- You can submit your most recent U.S. passport book and/or U.S. passport card with this application,
- You were at least 16 years old when your most recent U.S. passport book and/or U.S. passport card was issued,
- You were issued your most recent U.S. passport and/or U.S. passport card less than 15 years ago,
- You use the same name as on your most recent U.S. passport book and/or U.S. passport card, OR
- You have had your name changed by marriage or court order and can submit proper documentation to reflect your name change.
With this form you will submit 2 new passport pictures, your old passport & a check or money order for $80.00.
NAME CHANGE, DATA CORRECTION, AND LIMITED PASSPORT BOOK REPLACEMENT (DS-5504)
You may qualify to use this form only if:
- You have had a change in your name and your most recent U.S. passport book and/or U.S. passport card was issued less than one year ago;
- Your identifying information in your most recent U.S. passport book and/or U.S. passport card was printed incorrectly;
- Your most recent U.S. passport book was limited in validity and was issued less than one year ago.
With this form you will submit your most recent U.S. passport book and/or passport card, any required additional evidence, and 2 recent passport photos. There is no fee associated with the use of this form unless expedited service is requested. Your re-issued U.S. passport book and/or passport card and any documentary evidence submitted to Passport Services will be returned to you by priority mail.
